Timing was off again (very very off), I modified the depth of the needle but I have a hunch the bobbin cage is the issue since it was very loose. Definitely not working atm.

To fix it, we took off the front storage for thread (lift and roll to the right), then took off the bottom. Once the bottom was exposed, the holder of the bobbin was anchored with three screws to the shaft. By loosening these, the bobbin cage can rotate independently of the shaft. To align the timing, we put the needle at the lowest point and the claw of the bobbin cage just about to catch it. Once aligned, the three screws must be tightened while keeping the axle tight to maintain good gear contact. A trick to keep this easy was to only turn by hand in the "forward" direction (counter clockwise looking down on the dial). This keep the teeth of the gears in nice contact. Then the bobbin cage was held in a known place while the needle was rotated down its lowest. Finally, the three screws were tightened. Make sure to do some test sewing before putting it all back togetherr!

Industrial Sewing machine - Juki DLN-415-4

We have a Juki DLN-415-4 Industrial Sewing machine, with desk, but missing the oil reservoir/tray.

Found a manual at http://www.raichert.com/adobe/DLN415%20instruction%20book.pdf that says the oil reservoir/tray is necessary.

Based on this image of the oil reservoir, we may just be able to use something that simply holds oil over the intake.

The kind of oil is Juki Genuine Defrix Sewing Machine Oil No.1. However, we have Stainless "Lily White" sewing machine oil which looks similar and was used.
