[sudo-sys] Wiki user registration not working?

Sam was having trouble making a new user on our wiki. See:

It says you don't have permission. But how are people creating
accounts, then? I personally approved several new accounts last week.
I don't see any recent changes to localsettings.php except for
installing new extensions. Perhaps this new Wordpress integration
plugin has something to do with it?
