Yikes. There seems to be some very messy interactions
between multiple
old versions of a "ninja form" plugin. I don't know if this mess is
worth salvaging.
It's not, given that WPForms is so easy to use..
I recreated the membership form with WPForms and sent a little test
response a bit ago - seems to work fine!
